Payment Information
Evaluations are generally billed at a flat rate. The fee includes:
Parent intake (1-1 ½ hours)
Classroom observation (if warranted)
Administration (4-6+ hours) of standardized tests
Telephone contact with your child’s school (with signed consent)
Scoring and interpretation of standardized assessment tools
Written report
Parent feedback conference meeting (1 hour) to discuss results, make
diagnoses and recommendations
Feedback conference with the student (if warranted)
One related school meeting

Please note:
Dr. Walinsky is an out of network provider. Dr. Walinsky does not accept insurance. You may check with your provider for out of network psychological testing coverage. If you plan on submitting to your insurance company for reimbursement, you will be provided with the billing/procedural codes, diagnostic codes and a Federal Tax ID#.
The cost of evaluations can be paid for by funds from a Flexible Spending Account or Health Savings Account.
Dr. Walinsky can set up a payment plan to divide the cost of the evaluation into several payments. Please note full payment is due before the report is finalized and distributed.
